Problem With Feedback Form

Dear LP John,

Some Peace Rebels are reporting that the Feedback form is not working for them:

- In some cases the say that it only displays a white page

- Others say that after filling in all the information, once they submit it all the entries are lost and they get a notification that there are some fields missing even though they are sure everything was completed.
